Please review the form before filling out any fields and gather\r\n all your wage information prior to starting the Wage Claim form.\r\n You will need to provide complete contact information for the\r\n employer, the dates you are claiming wages for, the total amount\r\n of wages you are claiming, your date of separation and the date of\r\n the next regularly scheduled pay day following your separation.\r\n The form will not allow you to add or alter information once it is\r\n saved. Please complete all fields before saving. Incomplete forms\r\n will not be saved.
\r\nPLEASE BE ADVISED under most circumstances you cannot file a\r\n Claim for Wages until separation from the Employer has occurred.\r\n Once separation occurs Wyoming Law allows the Employer until the\r\n next regularly scheduled pay day to pay an employee everything\r\n that is owed. The Wage Claim Form will not save if you indicate\r\n you remain employed with the employer you are asserting owes you\r\n wages. Similarly, the Wage Claim Form will fail to save if you\r\n indicate a regularly scheduled pay day has not passed since\r\n separation.
\r\nAdditionally, the Wyoming Department of Workforce Services Labor\r\n Standards cannot go back more than two (2) years in pursuing a\r\n Claim for Wages.
\r\nPlease take a few minutes to review these simple steps for\r\n successfully submitting a wage and hour claim.
Please have all of your\r\n documents, supporting texts, emails, policies, etc. ready for uploading\r\n before you begin. You will have an opportunity to submit them at the end.
Please have all of the\r\n physical and mailing addresses associated with\r\n your claim available before you begin.
Please have all phone numbers and emails available\r\n before you begin.
You must complete the entire form in a single attempt because\r\n the database will not allow you to save it.
DO NOT click the SAVE button until you are ready to\r\n submit your claim; it does not SAVE; it submits the claim\r\n to Labor Standards, and you cannot return to the form.
Please\r\n do not leave any blanks empty or else\r\n the database will not allow you to submit the claim.
Once you believe you have completed the form,\r\n sign your initials on the bottom line.\r\n Then click VALIDATE.\r\n DO NOT CLICK SAVE yet.
Validating will review the form to see if\r\n any blanks were left blank or incomplete and will direct you\r\n to those areas that need to be completed or corrected.
Once you have made necessary corrections, click\r\n VALIDATE again. If all of your\r\n information is complete, you will see a message that instructs\r\n you to SAVE.
After saving, a window confirming your success will\r\n open, displaying your claim number and allowing you to\r\n UPLOAD your supporting documents.
